Individual Outreach Support
What support do we offer?
We support people with learning disabilities, autistic people, and people with other hidden disabilities, some of these people may also have physical disabilities or require mental health support. Some people we support have complex needs and require support for multiple aspects of their care.
We offer support in the community and at home for 4 or more hours a week. We are CQC registered which means that this can include personal care and support with medication. The people we support are adults and young people aged 11+. All our staff are DBS checked, experienced, fully trained, and carefully matched to meet individual needs.
What can you do with your support?
We are a member-led charity and always try to build skills that promote independence. We support our members to do a variety of activities. Below are just a few activities our members are regularly supported with.
Don’t see what you need? Just ask!
· Leisure activities such as going swimming, to the gym, bowling, playing football, and gardening
· Planning, shopping for, and cooking meals
· Help managing your tenancy or paying your bills
· Seeking and applying for work and/or benefits
· Building independent travel skills by using public transport
· Finding and making friends
· Dealing with a big change or event in your life from leaving education, to attending hospital, to bereavement
Paying for support
If you have been assessed for 1-1 support then it is likely Adult Social Care or Children’s Services will pay for the support. However, they will assess you for a financial contribution towards this which will be different depending on your circumstances. This will be communicated with you by Leeds City Council.
Some people pay via direct payments using a personal budget and others may pay privately.

We will then ask for some information about you and your support needs, and our service manager will arrange an initial meeting to get to know you further.
